The topic of this month's publication was chosen because of the prevalence of history, exam and workup problems as a contributing factor in NORCAL diagnostic error claims. This contributing factor prevalence is not unique to NORCAL policyholders. A recent study found that incomplete patient assessment was a contributing factor in half of the malpractice claims in its claims database.
