This article addresses two specific communication error scenarios that appear to be trending upward in NORCAL Group claims against radiologists. The first involves a radiologist’s failure to make recommendations for further studies in response to an ordering physician’s demands. The second involves failures to diagnose lung cancer following an incidental finding of an opacity, nodule, mass, or lesion on lung images, which should have been directly communicated.
